By clicking 'Continue', I approve the repair quote selected in this request for service. I agree to make full payment through ESS upon notification by Casio that repairs have been completed on my product, and understand that Casio will promptly ship my repaired product to me. I understand that Casio will hold my product for a maximum of 60 days from the date of original notification to me (with subsequent reminders) that my product has been repaired. I understand that if I fail to make the agreed payment within the stated 60 days, my product will be recycled as Casio sees fit with no further notice to me, and that my repair order will be closed out. I understand that in this event my product will not be returned to me and no further payment will be due from me for this repair request. If, upon evaluation by Casio's Repair Center, it is determined that the necessary service on my product exceeds the cost of the repair quote that I have approved, a revised quote will be issued to me. I will then have the option to accept or decline the revised quote. If I accept, the service request will proceed as above. If I decline this revised quote, I have the following options: (1) My unrepaired product will be returned to me after the payment of a shipping and handling fee ($8.95 for watches, $10.00 for digital cameras and $15.00 for large items). (2) My product may be recycled at my request with NO payment due, and my repair order will be closed out. If I fail to respond to this revised quote, my product will be held by Casio for a maximum of 60 days from the date of original notification (with subsequent reminders), after which my un-repaired product will be recycled with no further notice to me, and my repair order will be closed out. I understand that my product will not be returned to me and no further payment will be due from me for this repair request. I further understand that Casio will only ship to the fifty United States and U.S. Territories (including Commonwealth of Puerto Rico). If I live outside these areas, then I will arrange a shipping location within the specified States or Territories. In any case I must supply a physical address for FedEx delivery as Postal boxes may not be used. I also understand that Casio's Repair Center provides a 90-day warranty covering parts used in the repair of my product and the labor cost of repair. If at any time in the 90 days following receipt of my paid, repaired product, it fails substantially in the area of this repair request, it will be re-repaired at no cost to me (This determination is subject to Casio technical review.) except for the cost of shipping the product back to Casio Factory Service Center.
